I haven't so far, although it's a great goal to have.

I don't think so - I think I've mostly gotten over that.

        First name, no.  Last name, definitely.

        I do, but some people make it really hard to do

As a patient, in 2014.  With John, in 2015.

   Of course. Unfortunately, it happens more often as you get older.
